Personal Hygiene

  • Assist with bathing, grooming, dressing, and toileting.
  • Maintain privacy and respect dignity.
  • Support to use assistive devices (shower chairs, grab bars) to promote independence.

Mobility and Exercise

  • Help with safe transfers from bed to chair.
  • Support to use walkers, canes, or wheelchairs as needed.

Nutrition and Hydration

  • Prepare meals of your choice
  • Ensure regular hydration.
  • Address swallowing or chewing difficulties with soft foods or thickened liquids.

Medication and Medical Care

  • Keep a schedule and log of medications.
  • Assist with prompting medication as prescribed.
  • Monitor for side effects or interactions
  • Assist, monitor and treat tissue viability issues

Emotional and Mental Health Companionship

  • Spend quality time talking, reading, or engaging in hobbies.
  • Prevent loneliness, which can lead to depression.

Cognitive Health

  • Encourage brain-stimulating activities (puzzles, games, music).
  • Watch for signs of memory loss or dementia.
  • Keep a consistent routine to reduce confusion and anxiety.

Home Safety and Accessibility

  • Remove tripping hazards (cords, loose rugs).
  • Ensure proper lighting throughout the home.
  • Use non-slip mats in the bathroom and kitchen.

Promoting Independence and Dignity

  • Encourage the elderly person to do what they can independently.
  • Use adaptive tools for eating, dressing or bathing.
  • Listen to their preferences and involve them in decision-making.

Emergency Preparedness

  • Keep medical information and emergency contacts accessible.
  • Have a list of medications and allergies ready.
